How to make a proper cup of tea ( a brew!!)

What a drink. But how to make it ! Milk first, milk after? Sugar? Teabags or strictly leaves? How long to brew for?

Drink from China cup  only? Is it ever acceptable to drink tea out of a glass or plastic???
